We are excited to announce joint funding across North East London (NEL) for a 12-month pilot to support babies, children and young people (BCYP) requiring palliative care support at the weekends and bank holidays in a new Palliative care outreach nurse role. Two-part time posts have been created to work collaboratively across NEL, one based within East London NHS Foundation Trust (ELFT) as part of current Diana Children's Community nursing team, the second based within Haven House Children's Hospice.

Main duties of the job

We are excited to announce joint funding across North East London (NEL) for a 12-month pilot to support babies, children and young people (BCYP) requiring palliative care support. Two-part time posts have been created to work collaboratively across NEL, one based within East London NHS Foundation Trust (ELFT) as part of current Diana Children's Community nursing team, the second based within Haven House Children's Hospice.

This post holder will be based within the Diana Children's Community Nursing Team (ELFT base) and supporting care across North East London (NEL). This will be working alongside the outreach nurse based at Haven House Children's Hospice. The post holder will be required to work flexibly and be part of the on-call rota to support a child at end of life in the community across NEL. This post holder will be a peer support and require comprehensive handover between working to ensure continuity of care for BCYP and families, in collaboration with local Children's community nursing teams and hospices across NEL.

This post holder will be based within the Diana Children's Community Nursing Team (ELFT base) and supporting care across North Specialist Children and Young Peoples Service (SCYPS)

22.5 hrs over three-day weekend (Friday to Sunday or Saturday to Monday)
