Job summary

There are eight Neighborhood Mental Health Teams linked to the primary care networks within City & Hackney working with residents with severe mental illness and complex emotional needs. The model aims to take a population health approach to mental illness and includes close collaboration with local partner agencies, including the contribution of community connectors to the team to support residents to connect with relevant local resources and services. The Neighborhood Teams provide personalized and collaborative interventions to adults in their localities with severe mental health problems and complex emotional needs. Once the identified interventions to meet an individual's needs have been completed, they will be moved to the Inactive list on the team caseload. They can access support again via their GP, partner agencies or by directly contacting the Neighborhood. The focus is on residents being supported to achieve their identified outcomes, promoting social inclusion and recovery.

Residents with severe and enduring mental health conditions requiring more intensive, specialised or longer-term support can also access the Complex Needs Service for working age adults or the Mental Health Care of Older People (MHCOP) Team. Step-down Recovery Workers in the Neighborhood Teams can assess individuals needs and facilitate access to the Complex Needs Service if needed.

Main duties of the job

Each Band 7 Senior Practitioner within the Neighborhood Teams is responsible for the oversight of one of the teams, including management of caseloads and supervision and support to more junior staff within the team, chairing the daily referrals and weekly clinical meetings, and ensuring performance targets are met, as well as providing interventions tosome residents themselves. They will provide cross-cover support to their peers within the Neighborhoods. They will provide support to the Operational Lead for the Neighborhood MH Teams, including contributing to the development of policy and practice across the Neighborhoods and in deputizing for the Operational Lead in their absence.

Essential

  • 1.1 RMN, DipSW/Degree in SW or OT qualification

  • 1.4 ENB 998 or equivalent (nurses only)

Desirable

  • 1.2 First or Master's degree level or evidence of study toward qualification
  • 1.6 Post registration training in relevant area e.g. AMHP, practice educator, Non-Medical Prescribe

Experience

  • 2.1 Substantial community experience of providing mental health care at Band 6 or above
