Job summary

Accommodation Strategy Worker

The clinicians will work across the 4 neighbourhood teams with a focus on specialist out of borough funded clients and placements. This will be in close collaboration with the CCG, LBTH and neighbourhood mental health teams.

The post will be for a secondment period of 12 months. We will be ideally looking for a candidate who was able to start as soon as possible.

Candidates should have an interest in working with severe and enduring mental illness and high comorbidity, with clients from a range of religious, cultural and ethnic backgrounds. The post holder will be expected to work alongside other professionals from varying disciplines. The successful applicant must have experience of working in mental health (ward or community).

The post involves close working with the GPS, other partners, inpatient and community teams. This will also involve working with clients with varying severe and enduring mental illness with dual diagnosis, supporting community teams with required assessments (where indicated) and interventions.

Main duties of the job

Our Accommodation Strategy Team will provide high quality assessment and move on plans for people currently residing in residential care and out of borough supported living schemes with severe and enduring mental health problems, in partnership with service users, carers, local GPs, social care colleagues and voluntary organisations.

The service will offer a team approach applying concepts of Recovery to the service delivery. The team will provide a recovery focused, time limited service aimed at reducing social isolation, social exclusion and stigma, promoting and enabling the independence of people with mental health support needs, their integration into their local community and improvement in their quality of life. The team will provide oversight and delivery of an ambitious move-on pathway for service users.

The post holder will receive professional supervision and line management from the Operational Lead. The post holder works with the Operational Lead across ELFT and LBTH to provide a consistent, high quality service which is recovery focused, and sensitive to the needs of a diverse community with responsibility for ensuring service delivery is of a consistently high standard and assisting the team in meeting these standards.
