Job summary
The occupational therapist will work as a part of the multi-disciplinary Rapid Response/ Fast falls team to meet the needs of patients referred to the team, ensuring the highest possible standards of practice and the post holder will provide assessment and treatment for patients with a wide range of complex physical, sensory, perceptual and cognitive problems within community settings.
The Rapid Response/ Fast falls Team provides a service for adults requiring a more intensive level of case management, for short periods, as an alternative to hospital admission. The Rapid Response Team aims to prevent avoidable hospital admission and provide enhanced care and support to Nursing and Residential Homes.
Main duties of the job
To provide specialised client-centred occupational therapy intervention to clients (aged 16 or over) who are resident in the community and are referred primarily due to a physical disability. This includes clients who may present with complex physical, cognitive, psychosocial and housing needs as a result of a wide range of medical conditions.
To work as an autonomous practitioner in managing a defined caseload of complex cases, in a community setting. To evidence problem solving and clinical reasoning skills in line with evidence based and client-centred principles. To formulate a diagnosis and treatment plan designed to reduce the impact of disability, from an assessment of the client's needs.
To be an influential member of the multidisciplinary team, providing expert, integrated, client-centred, goal-orientated rehabilitation.
To provide supervision, teaching and appraisal for junior occupational therapists, support workers, students and other members of the multidisciplinary team
To work collaboratively with members of the multidisciplinary team and contribute to the planning, development and evaluation of the Extended Primary Care Services and Virtual Ward.
To continually engage with and contribute to the development of the provision of occupational therapy withincommunity services.
