Job summary

The Newham Health Team for Adults with Learning Disabilities (NHLD) is looking for a permanent, full-time (37.5hpw) Band 6 registered Learning Disabilities (RNLD), or Mental Health (RMN) Nurse to join our experienced Community Nursing Team. We are active in supporting services across Newham to ensure people with learning disabilities (PWLD) receive excellent health care and to reduce health inequalities.

Main duties of the job

The Nursing team works closely with our multi-disciplinary team colleagues which include Psychiatry, Psychology, Positive Behaviour Support, Occupational Therapy, Speech and Language Therapy, Art Therapy and Physiotherapy, to offer holistic assessment and interventions. Nursing is a key element of the teams clinical pathways and the post holder will be involved in the Challenging Behaviour Pathway ensuring that a cohesive, responsive community service approach is provided to patient care. The applicant must be able to work autonomously, prioritising and planning their own workload and be actively involved in decisions relating to clients with complex healthcare needs.

This post will primarily be based at the Newham Health Team for Adults with Learning Disabilities (29 Romford, Stratford, E15 4LY) and the Newham Centre for Mental Health, but expected to work across the borough and elsewhere as required.

The post holder will have a key role in supporting the continued development of the NHLD and work collaboratively other community health and social care professionals, in-patients (specifically the Newham Centre for Mental Health) and other key stakeholders to support individuals who require a specialist service response, aiming to maintain the individual within their community setting.

The post holder will provide a vital role in assessing the care needs and risks of referred individuals who require an emergency response, ensuring that treatment plans are developed, implemented and evaluated with a person centred focus.

The post holder will need to have a good working knowledge of the Mental Health Act, Mental Capacity Act, DOLs and CPA. They must also have a proven, clinical understanding of the underlying factors that can contribute to the development of mental health problems for PWLD, how symptoms manifest and the various risk management strategies, interventions available.

Experience

  • Applying evidence-based practice through the use of health assessment tools
  • Willingness to develop leadership and supervisory experience of junior Registered nursing staff and pre-registration student nurses
  • Experience at Band 5 or above. Must have some experience of working with adults with learning disabilities, preferably within community settings or inpatient settings

Knowledge

Essential

  • Awareness of equality and diversity issues within the context of inner London boroughs such as Newham

Desirable

  • Working knowledge of legislative framework that are relevant to clients who have learning disability and services pertaining to them
  • Ability to liaise with others and to use expertise as a specialist Nurse to work across professional and organizational boundaries
  • Ability to take the lead on health promotion/health education initiatives relevant to PWLD
