Job summary
We are looking for a superstar Social Worker to join a dynamic multidisciplinary integrated discharge team in Newham, on a 12-month secondment or fixed term contract!
This is a fantastic opportunity for a proactive, motivated, and enthusiastic individual to work alongside staff on the psychiatric inpatient wards in the Newham Centre for Mental Health, the Newham Home Treatment team and the Community Mental Health services and help provide an effective, person-centred hospital discharge service.
The individual in this unique role will provide expert social work advice and support to the psychiatric wards working with those in mental health crisis, have complex unmet needs and may face multiple disadvantages, as well as the undertaking of effective person centred statutory social care assessment for individuals and the development of robust discharge plans. Importantly, the individual in this role will provide valuable social worker input to the discharge pathway and provide links with inpatients and other statutory and non-statutory organisations.
You will have undertaken statutory work as per the Care Act 2014 and Mental Capacity Act 2005 and be able to apply other core legislation, such as, Mental Health Act 1983 / 2007, Human Rights Act 1998 and Equality Act 2010.
Strong assessments skills are a must, always using a person-centred and strengths-based approach with the ability to think creatively outside the box.
Main duties of the job
The post holder will:
Work within the multidisciplinary mental health integrated discharge team in Newham, on a 12-month secondment/fixed term contract.
Be employed by ELFT and seconded to the London Borough of Newham to undertake statutory social work duties on their behalf.
Work alongside staff on the psychiatric inpatient wards in the Newham Centre for Mental Health, the Newham Home Treatment Team and the Community Mental Health services, and help provide an effective, person-centred hospital discharge service.
Provide expert social work advice and support to the psychiatric wards working with Newham resident inpatients who have complex unmet needs and may face multiple disadvantages and, as well as the undertaking of effective person centred statutory social care for individuals and the development of robust discharge plans.
