Job summary
Would you like to be part of a progressive, forward thinking Community Therapy team for an exciting opportunity to make a positive impact on peoples lives through physical rehabilitation when discharged home from hospital? What about an opportunity to support the Ambulance services via an Urgent Care Response referral?
We are seeking a part time (22.5hours) Band 5 Physiotherapist, who is passionate and enthusiastic to join our Primary Care at Home service in Mid Bedfordshire. We are a 7 day service operating between 08.00 to 21.00 hours and successful candidates will work a variety of shifts.
This is an exciting opportunity if you are thinking about joining community therapy or gaining new experiences. You will develop strong assessment skills with a personalised approach, carry out therapy assessments working closely with other members of the MDT to deliver rehab to patients with complex needs within the place they call home. You will develop skills in moving and handling, risk assessing and identifying solutions to reduce risk to provide for the needs and personal outcomes of the individual, their families and carers.
Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. All applicants need to hold a degree/diploma in Physiotherapy and be registered with the HCPC.
It is essential that you are a car driver and have access to a car in order to carry out service duties.
Main duties of the job
- The post holder will be accountable to senior members of the Community therapy team providing support to clients for a variety of therapeutic and clinical interventions. This may include clients with diverse presentation, physical and psychological conditions.
- To work as part of a multi professional service, within the community setting, working with clients towards rehabilitation through taking an active role in the assessment, planning, implementation and individualised therapeutic interventions. (Access to Physio Tool is available to support the design of individualised exercise plans.)
- To assess, demonstrate and provide appropriate adaptive equipment to meet specific client needs and facilitate safety in their chosen environment.
- To hold responsibility for own caseload and work as an autonomous professional according to the Health Professionals Council's regulations.
- To develop good communication links with clients, carers and joint working with other services across professional and organisational boundaries in order to provide a client-centred, holistic and seamless service.
- To work a variety of shift patterns including weekends and bank holidays on a rota basis
Undertakes lone working in the community.
