Job summary
Our new Advance Care Planning team of nurses and occupational therapists aims to support people with their choices at end of life and to maximise their quality of life, including supporting them to be in their preferred place as much as possible. They work closely with the community health teams and colleagues in hospice and social care to have conversations about advance care planning so that people can die how and where they wish.
We are looking for a forward thinking, exceptional occupational therapist who is a skilled communicator to join our innovative Advance Care Planning team to deliver proactive, personalised, and coordinated care at every step of the way.

Main duties of the job
Provide clinical leadership and expert advice and support for the Advance Care Planning and Locality teams around occupational therapy assessments for patients nearing the endof life.2. Work autonomously to deliver high quality assessment and interventions for patientsnearing the end of their lives that meets the required standard, demonstrating advancedclinical judgement.3. Enable people approaching the end of life to receive timely personalised support for theirsocial, practical and emotional needs, which is appropriate to their preferences andmaximises independence and social participation for as long as possible.4. Support patients to choose their preferred place to die, as far as is reasonably possible.5. Ensure that families and others identified as important to the dying person are supportedand their needs are explored, respected and met as far as possible.6. Facilitate a high quality, cost effective specialist service through monitoring and audit andother quality initiatives.7. Be involved in the development, assessment and implementation of guidelines, protocolsand pathways.
