Unplanned Care Extended Primary Care Team
Are you are experienced band 8a nurse and looking for an opportunity to progress and work in a forward thinking team, that has the interest of patients in the heart of everything we do. You will be joining a team that has continually expanded with the aim of being an exemplary Rapid Response service in the east of London.
You must be a Registered Nurse (Adult) and have a or in the process of completing a MSc Level in Advanced Clinical Practice or equivalent experience with a non-medical Prescriber (V300) qualification.
We encourage our staff to progress and develop - you will be given regular supervision and appraisal and will be supported by the Service Manager and Lead Nurse.

Main duties of the job
We have an exciting opportunity for a forward thinking and innovative nurse to join us in the continuous development of our unplanned care team. This team comprises of the Rapid Response & Falls service (RR & FS) and the Integrated Discharge Hub (IDH). The FF & FS service had recently been expanded to fulfil the Urgent Community Response.

- You will provide senior management and leadership to a portfolio of nursing services

- Ensuring that nursing care is delivered to a very high standard with staff maintaining adequate competences
- Working with the Senior Management Team to achieve strategic objectives across community services
