Mid Beds Older Adult Community Mental Health Team has an exciting opportunity for a permanent Community Mental Health Nurse (CPN - Band 5). You will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the Operational Policy.

You will work as an autonomous practitioner within an integrated Multi-Disciplinary team which will involve the assessment planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the client. You will be confident and competent in administering IM injections and have a working understanding of the Medicines Management Guidance & associated policies.

Main duties of the job

To work as part of the Community Mental Health Team key-working a case load of people and delivering time limited intervention under the direction of the allocated Care Coordinator.

The post holder will facilitate, delivery and monitor individual interventions as defined in the individual , person centred plans of acre and within the principles of the agreed operational policy to deliver Personal Recovery and Social Inclusion. To support Senior Nurses in the team by administering and monitoring DEPOT in the community.

The post holder must hold a first level mental health nursing qualification and be registered with the Nursing & Midwifery Council.
