Job summary
We are looking for an experienced and enthusiastic healthcare professional - Registered Mental Health Nurse, Occupational Therapist or Social Worker who is ready for a new challenge to join our CMHT for Older People as Mental Health Liaison Practitioner, working as a link and liaison person with Primary Care Networks (PCNs), Community Integrated Mental Health Services (CIMHS), Social Services and other agencies as part of the Newham Directorate Community Mental Health Transformation programme to produce positive health outcomes and impact for older people with mental health problems.
This role is developmental and should be viewed as under construction since the older adult services are in the early stages of engagement with community mental health transformation.
Main duties of the job
- Work collaboratively with PCNs to facilitate older people with mental health problems' access to GPs and other specialist community health services and organisations.
- Be able to recognise potential crisis situations and act responsively and reasonably to resolve the situation.
- Conduct comprehensive telephone as well face-to-face- assessment of older people with sudden and ongoing mental health problems and provide advice to GPs and CMHTOP or signpost to other services as soon as possible.
- From comprehensive assessment, apply specialist knowledge and clinical skills to analyse the complexity and urgency of heath care needs and provide management advice to the referring team.
- Communicate effectively with CMHTOP and all those involved in the person's care, including family members where appropriate to ensure that the advice is based upon all the relevant information available regarding the person's presentation, background history and care needs.
- Aid GP Surgeries to deliver ongoing treatment at home to older people that are less able to attend GP surgeries and where appropriate to liaise with Social care Teams to facilitate assessment of social care needs that could reduce the number of unnecessary referrals to CMHTOP.
