Job summary

The Pathway Homeless Team works across the Royal London Hospital in-patients to provide advocacy, support and quality discharge interventions for homeless clients admitted to the Royal London Hospital. The Homeless Team works in close collaboration with the wider hospital discharge teams. The over arching aim of the team is to improve the health of homeless people.

Main duties of the job

This is an exciting post to help provide a now well established multi-disciplinary model of care for homeless people admitted to hospitals. The post holder will work alongside a specialist general practitioner & a Senior Homeless Persons Practitioner (Nurse), a Social Worker & a Care Navigator to provide additional support to homeless people.

Essential

  • oDiploma/Degree in Occupational Therapy.
  • oRegistered with Health Professionals Council
  • oPost Graduate Training

Experience

  • oPost-registration experience
  • oExperience working in a specialty involved with complex clients with problems homelessness or social exclusion

Desirable

  • oExperience of liaison with statutory and non-statutory services
  • oExperience of working in a multidisciplinary team

Knowledge and Skills

Essential

  • oClear evidence of understanding of the principles of clinical governance and its relationship to the role.
  • oDemonstrable evidence of knowledge of professional body Standards and Code of Conduct
  • oAble to demonstrate knowledge of best clinical practices relevant to the role
  • oKnowledge of the role and function of other organisations who provide services to clients who are homeless
  • oAble to use NICE and other national guidance
  • oAbility to demonstrate sound clinical judgement
  • oAbility to assess, plan and implement care for patients with supervision.
