Job summary
We are looking for an enthusiastic and skilled Community Mental Health Nurse with an interest in working in Stepney and Wapping Community Health Team that has a positive approach to supporting service users with their personal recovery.
The Tower Hamlets Stepney and Wapping Community Health Team is a well-established team within East London Foundation Trust and operates as an integrated multi-disciplinary team with staff from both the East London NHS Foundation Trust and the London Borough of Tower Hamlets working together to provide a high quality service for people with severe and enduring mental health needs this is inline with Transformation Project. The team actively works in partnership with service users, carers, the Local Authority and with other statutory services and local voluntary groups.
Main duties of the job
To be an active member of the multi-disciplinary team providing assessment, care and support to Adult presenting with mental health issues and / or cognitive impairment within the borough of Tower Hamlets.
To conduct comprehensive and high quality holistic Health and Social Care (Care Act 2014) assessments.
To act as a care co-ordinator for a caseload of service users and develop, implement and review individualised recovery focused care and support plans and commissioned packages of care.
To conduct carers assessments and offer support and advice to carers and
relatives.
To promote the needs of service users and offer and provide
training and education accordingly.
To operate as Assessment and Brief Treatment Team officer (ABT) in line with Transformation Project, responding to emergencies and processing referrals.
To assist the Senior Practitioners and the Operational Lead in the management of the service and deputise in their absence, where appropriate.
