Job summary
The post holder will support the UCR/VW multi-disciplinary team that provides clinical assessment and management of patients within the community setting team using independent judgement.
Responsible for the day-to-day operational leadership and management of the senior support staff, requiring the post holder to provide and receive complex, and sensitive information.
The team is multidisciplinary with an array of registered and unregistered professionals providing nursing, therapy and home support. The service offers holistic support to patients and families that may be experiencing a crisis and has the ability to offer digital monitoring for patients who are cared for upon the virtual ward. Our urgent community response service works in collaboration with our virtual ward and are one team working together to promote hospital at home.
Main duties of the job
Under the direction of the Service Leads, the post-holder will assist with the development and implementation of projects and initiatives including maintaining appropriate documentation, ensuring projects and service development are delivered within time and budget constraints.
Develop positive working relationships with managerial, non-clinical and clinical staff across the Business Group and Trust and all partner agencies.
Ensure all staff are fully engaged in all developments. This will require a very high level of communication across all staff groups using regular team meetings as one forum for this.
Provide the necessary leadership and drive to ensure that agreed developments are implemented in a sustainable way
Link in with other initiatives, communicating potentially sensitive information about required changes and quality standards
Contribute to the coordination and gathering of information for strategic investment proposals, business cases and options appraisals as and when required.
