The Children's Continuing Care Team, which provides commissioned care in the home and school setting for children with complex health needs.The Long Term Ventilation Team which provides packages of care in the home for children who require ventilation to assist with their breathing.Swanbourne Gardens which is a 4 bedded respite unit that provides overnight respite for children with complex health needs.We strive to provide the children we support with a high standard of care. As a band 5 working within the team you would be responsible for the assessment, planning, implementation and evaluation of patient care.
You would be at the forefront of teaching and assessing the competencies required for the staff to deliver day to day care for the children with complex health needs, this will include interventions such as care and management of tracheostomies, gastrostomies and ventilation support. In addition you would be expected to provide supervision for the junior staff.
As a band 5 we would expect you to role model thetrust's value and behaviour framework when working within the team.Robust training and regular competency reviews will be provided to ensure care is provided safely and in line with all policies and guidelines. The successful candidate will be based at Swanbourne Gardens, but will provide care in other community settings such as the child or young person's home or school.
You must have a full driving licence and access to a vehicle for work purposes.
Main duties of the job

To undertake a range of duties, which involve direct care, following care plans agreed with parents and named professionals whilst working in the children's home, school or other environment where they spend time.
To be aware of, and participate in meeting the psychological, social, physical and spiritual needs of the children and young people.
Maintain accurate records of all care given, omissions and ensure effective communication with parents and named professionals of any changes in a child's condition or situation, and any significant relevant factors.
To promote professionalism at all times.
We are looking for staff that are able to be flexible,we work around the needs of the families and the children which means we need staff who can flex their hours to start early, finish late and work overnight too.
We are looking for an Band 5 Paediatric Nurse. The post holder will have post registration experience, preferably of working with children with complex health needs, either in a hospital or community setting.
Full training and supervision will be available from our existing senior team, but any previous experience would be desirable.
We are a very small but supportive team who can offer support and guidance to you in developing new skills and adapting to working in a new environment.
