Job summary

A fabulous secondment opportunity has arisen for a band 5 Diabetes Nurse Educator to join our dynamic integrated specialist diabetes team in Stockport.

This post will be perfect for a nurse with at least 12 months experience who wants to pursue a career within the dynamic arena of diabetes. We welcome applications from nurses who enjoy delivering education and have exceptional communication skills. We are a cohesive team who work closely with Consultants and Primary care colleagues alike. We pride ourselves on providing first class care to our people with diabetes.

You will be working in a specialist team and will have a role both with the inpatient diabetes nurses and out in the community seeing patients both in clinic and their own home.

Main duties of the job

  • To assess, plan, prioritise, implement and evaluate nursing interventions to meet the patients' needs which may be in the community or acute setting in relation to speciality of diabetes.
  • Support the Diabetes Specialist Nurse team to provide an evidence-based nursing service, which is responsive to the user's need. Care will reflect current local protocols and evidence based national guidelines and will be delivered through face-to-face consultation or via telephone communication on a day-to-day basis.
  • Review care and progress informing diabetes specialist nurse of progress
  • To advise, educate and support clients and carers on the management of diabetes by providing specialised programmes of care in relation to health and illness and the development of self-management techniques including individual and group sessions under guidance from diabetes specialists
  • Facilitate discharge following plan from diabetes specialist
  • Ensure efficient running of individual and group education
  • Responsible for the completion of identified audits by the specialist team
  • Adhere to Nursing & Midwifery Council (NMC) Code of Conduct and associated guidelines.
  • Maintain confidentiality in all aspects of care and employment
  • Act as a role model at all times.
  • Maintain relevant professional registration.
  • Maintain appropriate records adhering to Trust and Nursing &Midwifery Council (NMC) guidelines on record keeping
  • Facilitate discharge following plan from diabetes specialist.
