Experience Based Co Design Project Facilitator - Band 6 (Indicative and subject to AFC evaluation)

Fixed term Contract - 9 months (some flexibility may be considered)

An exciting opportunity has arisen within the Pain Management Service to lead on the development and delivery of a patient experience based co design project to transform the Stockport pain management pathway.

We are looking for a dynamic Band 6 Experience Based Co Design Project Facilitator who are highly motivated and innovative to join a dynamic team and lead on the 'Your Pain Journey' Project.

Successful applicants are required to have experience of patient co-production and project management in a healthcare setting. They will need to be organised, show enthusiasm, demonstrate excellent communication and time management skills to ensure high standards are continuously met.

Main duties of the job

To lead on the development and delivery of a patient experience based co design project to transform the Stockport pain management pathway within the 'Your Pain Journey' project.

To provide specialist patient experience based co design expertise to the project, working alongside our Trust's Transformation Team to ensure project timeframes and objectives are met.

To facilitate patient engagement events, gain patients view through multiple channels and collate patient opinions to develop a holistic patient centred model utilising a variety of community services, volunteer partners and primary care. In addition, help create and develop resources (written and/or electronic) for patients living with Chronic Pain and their families/carers.
