Caseload Coordinator - some weekend working required

This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.

UCR/VW is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis The service can clinically monitor patients remotely via Virtual Ward/Hospital at home using enhanced technology which is supported by a Digital Health team.

Main duties of the job

To undertake tasks which involve the management and maintenance of the team's clinical caseload for Senior Support Workers (SSW) including all associated administration tasks.

The job will include using a variety of computer-based programmes including EMIS, Advantis and Microsoft Packages.

Work closely with the Multi-disciplinary Team to coordinate patients care.

Responsible for delegation of visits and coordination of the SSW daily work

Oversee the distribution and collection of the electronic devices to and from patient's own home
