Job summary

This newly created post is an exciting opportunity to play a part in Patient Safety at Stockport NHS Foundation Trust. You will have responsibility for the risk and safety team providing leadership to the team members that oversee claims and inquest administration, incident management, safety alerts and subject access request responses across the Trust and also have the opportunity to bring this all together and support with the transition to the NHSE Patient Safety Incident Response Framework.

As well as being able to lead a team through a time of change, it is also a great opportunity for someone with strong project management skills. You'll be a key member of the Patient Safety Incident Response Implementation Group and will support the Deputy Director of Quality Governance and Head of Quality Governance with oversight of workplans and communications regarding requirements of PSIRF. The role will also lead on supporting the annual production of the Quality Account for the Trust. This is a great opportunity for the right individual to take on a challenging but rewarding role focused upon patient safety and improvement.

The Patient Safety Manager will be based in the Risk and Safety Offices on the Stepping Hill site. We are a supportive team and you'll also work closely with the Patient and Customer Service Team who manage our complaints and PALS enquiries and the Divisional Governance Teams.

Main duties of the job

  • Lead on the development and review of the Patient Safety Incident Response Plan and Policy with the Head of Quality Governance and Deputy Director of Quality Governance
  • Ensure effective and appropriate teaching of patient safety to all staff working in the organisation, utilising where appropriate national and regional offerings
  • Support the Risk and Safety Team Leader to ensure the Risk Management System is fit for purpose
  • Be responsible for liaising with all Divisional Governance Teams to gain assurance regarding patient safety requirements
  • Support an integrated approach to all aspects of quality and safety, ensuring robust systems to support
  • Engage clinical and non-clinical staff to maintain the profile of patient safety ensuring that related strategies are embedded within the culture of the Trust
  • Contribute to and support implementation of patient safety policies related to risk and governance for the organisation, including but not limited to incident policies, duty of candour policies, inquest and claim policies
  • Undertake analysis and presentation of patient safety information from a number of sources to identify trends, themes areas for local action and learning
  • Continuously promote and prioritise a safety culture, acting upon learning and evaluation of changes and improvement
  • Project management the submission of the Trust Quality Account each year working with colleagues across the organisation to produce the account in line with national timescales

Planning and organising

  • Plan and organise own workload and that of the teams you are responsible for, adjusting work plans as required, and escalating priorities and concerns to the senior team
  • Coordinate reports to groups and committees as work plans require, including the quarterly patient safety report, and monthly notification of serious incidents report (this will transition to a PSIRF report in due course)
  • Ensure a robust process is in place in relation to incident oversight, claim and inquest preparations offering support to the team to problem solve and ensure best outcomes for the organisation and our patients and families
  • To work directly with our Divisional Governance teams to ensure implementation of the Trust incident reporting system, supporting where appropriate any investigations
  • To lead the Risk and Safety Team and ensure that the Risk Management System is regularly updated as new upgrades are released and that staff are trained to use the system
  • The lead on the continued development of Patient Safety Incident Response Framework implementation and support the Deputy Director of Quality Governance and Head of Quality Governance to design a fit for purpose Patient Safety Incident Response Plan
  • To monitor effectiveness of the PSIRP and support the continued review and update in line with national requirements
  • To ensure that all patient safety processes are in line with national and regional requirements following direction from the Head of Quality Governance
  • To facilitate designated groups including the incident review group and serious incident review group and to lead changes in line with the transition to PSIRF
