Job summary

This is a new role to support the delivery of a robust surveillance pathway for Hepatocellular Cancer for local West Herts patients.

The successful applicant, under the direction, guidance and supervision of the Hepatology and HPB CNS team and Consultants, will coordinate the care and monitor surveillance pathways for patients with cirrhosis enrolled in Hepatocellular Cancer Surveillance at West Hertfordshire Teaching Hospitals NHS Trust. The post holder will also have a specific focus to provide direct support for patients to maintain engagement in surveillance pathways ensuring they receive surveillance scans on schedule.

The post holder will participate as a valued member of the Hepatology team, assisting with specific duties and taking responsibility for managing a delegated workload within the service under specialist nursing supervision.

In addition, the post holder will support hepatology service with the delivery of high standard, efficient and safe Fibroscan for patients with liver disease. Fibroscanning (Transient elastograpy) is a non-invasive investigation which measure liver fibrosis and have increasing utility across the hepatology service.

This is a full time position with working hours Monday-Friday (Cross site working required between all sites - Watford, Hemel Hempstead and St Albans hospitals)

Main duties of the job

  • To identify patients that should be on the Liver Surveillance Pathway (LSP) that are not currently listed, including those that have failed to attend and / or are lost to follow-up
  • To populate the mandatory field template in relation to the Eclipse portal to ensure the named patient has been added to the portal with all relevant details
  • To work with the hepatology service to ensure all patients that should be listed on the LSP registry are added appropriately
  • Ability to access local hospital patient administration systems and data to identify appropriate patients
  • To add newly identified patients to LSP as identified by the local service
  • To contact patients that are lost to follow-up or disengaged with LSP and support them back into the surveillance programme, send blood forms, remind patients of the importance of having the investigations ahead of hepatology appointment
  • Effectively deliver Fibroscans to a high standard and in accordance with local and national training protocols (training will be provided)
  • Support the team in delivery of high standard, safe and efficient Fibroscan service
  • Work effectively with the hepatology multi-disciplinary team
  • To contribute to the quality assurance process departmentally and undertake audit and quality improvement as required
  • Act as a liaison for all healthcare professionals involved in health care provision to facilitate integrated patient focused care
  • To treat all patients with dignity, respect and high standards of care
