Job summary

An opportunity has arisen for a Clinical Fellow in Department of Anaesthesia and Critical Care to join our friendly team helping to deliver the exceptional care we pride ourselves in.

The Initial Assessment of Competency in Obstetric anaesthesia and Primary FRCA (or equivalent) are essential and the postholder will be expected to take part in the on call rota.

Main duties of the job

The Clinical Fellow will work alongside the other anaesthetic trainees within the Department of Anaesthesia and intensive care. There will be valuable experience in the day to day running of a large anaesthetic department. The postholder will be expected to attend directorate and faculty meetings and it is hoped they will play a full part in all discussions. All doctors will receive instruction and training as appropriate to their needs.

The Clinical fellow will be expected to exercise an intermediate level of clinical responsibility but ultimate responsibility for the patients they treat will rest with the relevant consultant. The post holder will work on 1:8 on call rota pattern and form one member of a full shift, first on call for ICU, Theatres or as SRA (Senior Resident Anaesthetist).

Job responsibilities

Critical Care Service - Watford General Hospital The 18 bedded combined ICU / HDU was opened in March 2009 and is fully equipped to manage up to 18 Level 3 patients. The unit provides critical care services to all departments in the trust, and has a 7 day a week Outreach service and a consultant led follow up clinic. The hospital has a busy Interventional Cardiology Suite and the ICU manages patients on Intra Aortic Balloon Pumps with consultant cardiologist support.

The unit is an active member of East of England Critical Care Operational Delivery Network The unit has been enrolled in ICNARC since June 1997; there is a full-time critical care audit nurse and full-time ward clerk support. There is an active audit and research programme and we have recently finished an RCT as well as 3 portfolio studies. The ICU is staffed by 2 consultants, along with Specialty Doctors, Anaesthetic trainees, ACCS and Core medical trainees. One of the consultants leads the outreach team.

There are multidisciplinary Consultant led ward rounds at 08:00 and 17:00 and a joint round with a Consultant microbiologist. There is an excellent team of highly trained and dedicated nursing staff, with a nurse practice facilitator to aid with nurse training and skill mix.Obstetric Anaesthetic Service - Watford General Hospital All inpatient maternity services for West Herts NHS Trust have been sited at Watford General Hospital since May 2002. The number of deliveries is over 5,000 a year. We have a regional block rate of approaching 40% and a caesarean section rate of 30%. There are two obstetric theatres, 2- bedded Obstetric Observation Bay in addition to the delivery rooms in the Consultant led obstetric unit. In addition, there are 8 delivery rooms in the midwifery birthing centre on the floor below.

There is an elective caesarean section list each weekday morning and an anaesthetic clinic on Monday afternoon when all patients for the lists are seen as well as any patients who wish to speak to an anaesthetist antenatally. There are currently 30PAs of consultant anaesthetist time dedicated to the unit. An ODP is available to help the anaesthetist with all the theatre cases. Midwives assist the anaesthetist to site regional blocks for pain relief in labour and we are able to provide minimum motor block epidurals.Chronic Pain This service is provided across the Trust by 5 Consultant Anaesthetists as part of a multidisciplinary team with psychology and physiotherapy. Out-patient clinics are held on both sites, while procedures are done mainly at St Albans City Hospital as day patients. New developments include a multi-disciplinary one stop clinic at St Albans.Academic facilities There are Post-graduate Centres on all 3 sites. At Watford General Hospital the lecture theatre has been recently upgraded and refurbished and a new simulation centre was opened in 2016. All have well-stocked libraries with a full range of journals run by professional librarians. There is an on line literature search service, and wireless internet access throughout the Trust. Each site has a full, well planned educational programme for primary care and hospital staff under the leadership of the Associate Medical Directors. These include weekly meetings targeted at general practitioners and hospital grand rounds on Fridays.Continuing Professional Development The Trust actively encourages CPD/CME for all staff. The current budget level for each specialty doctor in the directorate is £250. The successful candidate will have a yearly appraisal. Between appraisals Dr Kaur advises regarding CPD for specialty doctors particularly with clinical audit projects. Each doctor will have a documented professional development plan with clearly identified aims such as ALS provider, training to be a trainer or pursuing an ST training post. Study leave will be a maximum of 30 days in any period of 3 years. The Clinical Fellow will be fully supported in research that they wish to undertake that is consistent with the clinical priorities of the service and Trust.
