Job summary

The discharge lounges support the flow of hospital by supporting earlier discharge to create capacity in the inpatient clinical areas for patient in our ED who require admission. The role includes working with other AHP to ensure patients leave hospital safely and with their discharge notifications and medications and that all relevant support agencies are aware that our patients are leaving hospital.

Main duties of the job

The role includes working with medical teams for the completion of discharge notifications, pharmacist and pharmacy teams for medications and providing patients with a full explanations of their discharge documents.

The role will involve effective communication between different teams and with care agencies / patient transport providers and relatives and carers to ensure safe discharges.

The lounge provide abilities and capacity to give last dose IV antibiotics to support increased capacity on the wards. The lounge at Tunbridge Wells supports simple TTO dispensing to help elevate pressures within our dispensary and allows patients to level hospital in timely manner and prevent long waiting in the lounge.

Tunbridge Wells Hospital opened in 2011 and provides single rooms with en-suites for all in-patients - the first of its kind in the country.

We employ over 6,500 full and part-time staff in a huge range of clinical and non-clinical roles across both of our hospital sites.

Following your Corporate Induction you will be expected to attend the Trusts 2-week Induction Programme for RNs, NAs, RMs, ODPs and Paramedics. The programme is full-time (please email mtw-tr.professionalstandards@nhs.net if this is problematic)and will involve travel to both Trust sites. The programme includes supernumerary shifts in the clinical area, classroom-based sessions and e-learning.
