Job summary

Ward 31 is based in Tunbridge Wells Hospital and is a 30 bedded unit for both male and female patients, specialising in trauma orthopaedic nursing. We work collaboratively with a range of specialties to facilitate clinical excellence and holistic care delivery with a multi-disciplinary approach.

We have an exciting opportunity as we are looking for a Flow Coordinator to work with us Monday to Friday. This post will play an important role in ensuring there is a safe and smooth transfer of care from our wards to a variety of settings including other hospitals, the patient's own home and a range of other community settings.

The post holder will use their own initiative to support the flow of the ward, develop new processes and identify areas that could reduce the non-clinical work load for the nursing staff.

Main duties of the job

The post holder will provide proactive and responsive practical and administrative support to ensure the efficient use of capacity and timely patient flow within and out of the ward. They will support efficient and seamless ward processes from admission to discharge, concentrating primarily on facilitating discharge actions.To support the Ward Manager and 'Nurse in Charge' by facilitating board rounds and being responsible for monitoring all actions required, ensuring they are taken in a timely manner to provide a seamless service for patients through the admission, transfer and discharge process.

To be responsible for the data input into clinical systems that support flow and timely discharge of patients.To assist with simple discharges and support the IDT team by using effective co-ordination and communication skills to enhance the discharge process; including completion of CHC, Fast Track and Home First referrals for authorisation by a Registered Practitioner.In supporting the flow of the ward, the post will entail non-clinical tasks and administrative tasks under the supervision of the Nurse in Charge.The post-holder will be required to provide a link between the ward and internal services such as Site Managers, Matrons and the discharge lounge to achieve Trust targets, providing administrative assistance and ensuring effective communication between Multi-disciplinary Team members.
