We are offering 2 exciting, innovative full time posts working across the Leicester, Leicestershire Rutland interface within the University Hospitals of Leicester (UHL) NHS Trust Cardiorespiratory and North West Leicestershire Primary Care Network (NWL PCN) pharmacy teams.

- To deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring and monitoring compliance.
- To support the CMG Lead Pharmacist in maintaining and improving the pharmaceutical service to the clinical management group (CMG).
- To be responsible for the delivery of an agreed standard of pharmaceutical care to identified patients in order to achieve the safe and cost effective use of medicine.
Main duties of the job
You will utilise your clinical experience by providing daily clinical pharmacy services to specified wards, associated areas, assisting with the training and development of others and supporting the improvement of the services.
You will work closely with the multidisciplinary team to help maintain the high standard of services already provided. There is an ever growing number of Independent Prescribers in the teams and a strong desire to increase these numbers further so that responsibility for prescribing becomes integral to a pharmacist's roles.
You will have the opportunity to develop your clinical knowledge and expertise within respiratory and cardiology. Our team has a strong focus on medicines optimisation. We have well established links with our specialist clinical teams and work closely with them to support patients.
Post holder will be expected to contribute towards evening, on-call, weekend and bank holiday rotas.
NWL PCN
The Pharmacist will work as part of the general practice team to improve value and outcomes from medicines. This will include the management of long term conditions and advising on poly pharmacy. We offer support from a designated medical prescriber and our dedicated Senior Pharmacist team alongside the wider team. We enrol those new to general practice onto the Centre for Pharmacy Postgraduate Education pathway for General Practice and support our team to become confident and effective independent prescribers.

NWL PCN was formed on the foot print of NWL Federation. It has 109K patients across 12 practices, that work cohesively together. The pharmacy team consists of over 20 Pharmacists and Technicians. Pharmacists have been a success story in our PCN. We pride ourselves in our innovative working, progressive thinking, and consequently have our pharmacy team leading in many projects including women's health and respiratory care. - To participate in the delivery of optimum pharmaceutical care to patients served by the clinical directorate on specified ward(s) / clinics and deliver a level of clinical input commensurate with the grade.
- To contribute to the management of the directorate drugs budget via regular expenditure analysis feedback and participate in structured ongoing drug use audits to ensure optimal drug usage.
- To provide education and training to all other healthcare staff as appropriate
- To participate in late duty, weekend and bank holiday cover in accordance with departmental rotas.
