Diabetes Development Nurse

Job summary

The diabetes department at the University Hospitals of Leicester (UHL) NHS Trust has exciting opportunities for a Band 5nurse to join the existing nursing team and experience a development programme becoming a band 6 Diabetes Specialist Nurse dependent on successful completion of competencies and specialist education programmes.

The successful candidates will experience working in inpatient and outpatient areas, working alongside the multidisciplinary team and participating in the delivery of integrated care across the hospital trust.

Main duties of the job

To work in collaboration with other health care professionals within the diabetes team and wider team within the hospital trust

To work in collaboration with patients and their carers to develop individualise care plans providing knowledge and support enable them to self-manage their diabetes

To provide support for all grades of nursing staff in relation to diabetes nursing and to ensure a safe framework for practice is in place and maintained across the trust

To work with patients in different settings, (i.e inpatient and outpatient) providing a service that is sensitive to the needs of the individual person with diabetes

In return, they will be supported by the Senior Diabetes Specialist Nurses and the wider multidisciplinary team: to provide specialist advice , support and education to the patient, relatives and careers. There will be opportunities for continuing professional development through different sources.

The predominant focus of the post holder will be to assess, implement and evaluate diabetes patient care. In addition, the role will involve attending specialist clinics and providing education to healthcare professionals. These vacancies include working in our pump service, diabetes virtual ward and inpatient and outpatient services.
