This is an exciting opportunity to join Leicester Hospitals in a newly funded Advanced Pharmacist Practitioner Frailty role. Given this is new to the organisation, you will be working closely with key stakeholders across several clinical areas, patient safety and primary care on both trust and system wide initiatives. Key contacts will be our Frailty and Geriatric Medical and Pharmacy teams.

An independent prescribing qualification will be required for the role, or a commitment to work towards one. Key to this role is collaborative working and excellent communication skills, good insight into clinical governance related to frailty and pragmatic solutions to complex problems.

This role is an 8b Advanced Pharmacist Practitioner post and will have a bespoke job plan.

Main duties of the job

  • The post holder will be responsible for the provision of a highly specialised pharmacy service to patients across the Trust.

  • They will be expected to provide expertise to nursing and medical staff (within the Trust and externally) on the management of individual patients with frailty.

  • They will be expected to integrate with the relevant clinical teams to ensure efficient transfer of clinical and medicines management issues.

  • They will take overarching responsibility for all pharmacy research and audit projects within the service and be responsible for overseeing training including undergraduate placements, trainee pharmacists and diploma including independent prescribing.

  • Member of professional organisation e.g. RPS, BGS or UKCPA
  • Registered practicing Independent Prescriber
