Your primary base will be Glenfield Hospital however travel between UHL's hospital sites will be required; including the National Centre for Sport and Exercise Medicine (NCSEM) Loughborough, Aylestone leisure centre, and other community sites across Leicestershire as well as your primary base. If you're initial location is one of these sites excess travel reimbursement would not apply for a permanent or temporary change of base.

Main duties of the job
Job Summary
Provide clinical and non-clinical support to the Cardiac Rehabilitation team under the direction of a Cardiac Rehabilitation Specialist.
- To support the delivery of Cardiac rehabilitation interventions for the outpatient Rehabilitation services, under the direction and supervision of a Cardiac Rehabilitation Specialist across the Glenfield, Leicester General Hospital and Loughborough University (NCSEM) sites within UHL or community sites across Leicestershire under the direction and supervision of a Cardiac Rehabilitation Specialist.

Job responsibilities
Undertake specified direct and indirect care, by delivering safe and effective rehabilitation classes to individuals who have the diagnosis of a Cardiac condition.
Deliver unsupervised specified care and intervention as prescribed by an appropriate Registered Health Care Professional in accordance to agreed Trust and Directorate policies, procedures and protocols i.e. exercise prescription.
Assist the Registered Professional by developing care plans, assessing and documenting care, providing verbal and written reports in an effective and safe manner.
Make observations of care with regard to supporting the patient/users general health and well-being i.e. patients response to exercise
Maintain effective communication with the Cardiac Rehabilitation team regarding issues relating to assigned duties, promptly reporting areas of concern
Assist and support care delivery by only undertaking delegated work that is within own level skill and competence.
Contribute to the overall effectiveness of the care delivered by the Cardiac rehabilitation team and that benefits the patient/ user experience i.e. by promoting independent exercise and long term maintenance of appropriate exercise and healthy lifestyle behaviours.
Carry out duties consistent with individual patient / users wishes, maintaining privacy, dignity and recognising cultural diversity and needs.
