Job summary

The Patient Safety Co-ordinator is a valued member of the Women's & Children's Patient Safety Team.

This role will be responsible for supporting the Quality & Safety Manager in the improvement of patient safety in all aspects of clinical practice and service delivery.

The post holder will be responsible for implementing all requirements in relation to incident management acting as a case manager/lead investigator and ensuring performance and quality standards are met.

There are two full time posts available.

Main duties of the job

Provide advice and support to the Clinical Management Group (CMGs) and others on all issues relating to patient safety (e.g. incidents and complaints).

Support the Quality & Safety Manager by reviewing Datix-web to ensure all clinical incidents are reviewed and graded appropriately and within the required time frames.

Support, advise and as required engage with the CMG to ensure compliance with the requirement to inform patients/relatives (Being Open/Duty of Candour).

Lead investigations, (wherever they originate) ensuring the use of Patient Safety Investigation tools and write findings/report identifying achievable actions that transfers in to individual and organisational learning.

Support the referral of relevant patient safety incidents to external organisations, including collation and provision of relevant documents to these organisations, liaising within and outside of the Trust and ensuring that robust action plans are developed in response to reports received.

Communicate with patients and their families in a sensitive manner to advise about patient safety incidents.

Undertake Rapid Reviews of clinical incidents that meet the threshold for this and ensure that appropriate levels of scrutiny are applied and that escalations of concerns are progressed if indicated.
