BASE: Base site LRI/LGH/GH
HOURS: Up to 37.5 hours per week (plus weekends, bank holidays and on call services as per departmental rotas)
REPORTS TO: CMG Lead Pharmacist
ACCOUNTABLE TO: Chief Pharmacist

To deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring and monitoring compliance whilst in the relevant CMG rotation. To support the CMG Lead Pharmacist in maintaining and improving the pharmaceutical service to the clinical management group (CMG). To be responsible for the delivery of an agreed standard of pharmaceutical care to identified patients in order to achieve the safe and cost effective use of medicine.
