Job summary

We are looking to recruit a qualified and experienced LD nurse to join the UHL Learning Disability Acute Liaison Service The ideal candidate will have excellent communication skills and be able to demonstrate effective working with multiagency colleagues and advocate for the needs of patients with learning disability.

The post holder will report to the Lead Specialist Nurse for learning disabilities. .The post holder will provide a service across all sites and CMGs of the Trust. Whilst the Acute Liaison team predominantly works Monday to Friday 8am to 4pm the successful candidate may need to work flexibly on occasions.

Applicants wishing to apply for this post should be a Registered Learning Disability Nurse with relevant post registration experience at Band 5 or above. Consideration may be given to other non LD Registered nurses who can demonstrate significant post qualifying experience of working with adults with learning disabilities. Evidence of ongoing professional development is essential

Main duties of the job

The primary focus of the role will be to support acute hospital staff in the effective delivery of person centered care to patients with a learning disability. As a first point of contact the post holder will provide expert advice, professional support, guidance and education to the multi-professional teams, partner agencies, patients and carers. You must have the ability to work both as part of a team and autonomously, have excellent interpersonal skills and be able to prioritise and manage your own workload. You must be committed to advocating for the rights of vulnerable people and act as a role model at all times.

The post holder will ensure the delivery of high quality, clinically effective care, utilising research, and collaborating with colleagues in everyday practice and work in partnership with the MDT to enhance professional standards of care across the speciality.

The post holder will deliver training regarding the needs of patients with learning disabilities to all levels of staff employed by the Trust.

The post holder will work in collaboration with mainstream hospital services to enable open and easy access to healthcare services for people with learning disabilities.

The post holder will work alongside health professionals and senior hospital staff to achieve a positive experience and clinically appropriate outcomes, for patients with a learning disability.

The post holder will be a key player in the implementation of the hospitals action plan for people with a learning disability, including attendance at the hospital Learning Disability Steering Group
