Job summary
This Fellowship will support the integration of a new virtual CKD service in Leicester, Leicestershire and Rutland. The Leicester, Leicestershire, and RUtland Chronic Kidney Disease Integrated Care Delivery Project (LUCID) was developed in 2021/22 and has been piloted in 2022/23 in four Primary Care Networks. Applications from primary care professional such as GP specialist registrars and community pharmacists are strongly encouraged. Previous experience of working in a secondary or tertiary care kidney centre is not required.
The model has been developed such that Consultant Nephrologists will have responsibility for between 3-4 primary care networks in the region and meet regularly weekly with representatives from these groups to review patients on the CKD register, or patients with new presentations of kidney disease to review investigations, streamline management and fast track referrals for secondary care review and investigations. This has been developed in response to the latest NICE guidelines [NG203] which recommended referrals be based on 5-year risk of developing the need for dialysis or a kidney transplant using the Kidney Failure Risk Equation (KFRE). To implement this successfully will require dramatically improved rates of albuminuria screening which is required to calculate the KFRE and upskilling in the knowledge and implementation of pharmacotherapies to slow progression of CKD, such as ACE inhibitors and SGLT2 inhibitors.
Main duties of the job
This is a 0.4 FTE Fellowship post, and candidates will be required to discuss the remainder of their clinical job plan with their current employer. It is anticipated that the Fellow appointed to this post will work with the LUCID team and Primary Care Networks to:
- Develop educational packages to:
- Improve albuminuria testing in primary care, particularly the engagement of patients from ethnic minority backgrounds and lower socioeconomic status
- Improve prescription and optimisation of medications
- Work with primary care physicians and allied health professionals (nurses, pharmacists, dieticians) to support the implementation of this education
- Participate in LUCID clinics and education sessions
- Perform evaluation of the education packages, with the support of the LUCID evaluation workstream

There are modern laboratory facilities in the University of Leicester Medical Sciences Building within the Dept of Infection Immunity and Inflammation, with ~25 full time academic staff, research fellows and post-graduate students. Several research staff are supported by grants from Research Councils, Charities and from industry.
The department is committed to increasing the amount of clinical research and is currently recruiting to several UK nephrology studies which are part of the NIHR renal portfolio.
TEACHING AND TRAINING
The department strongly supports education and training of junior doctors and it is vital that there is sufficient staff within the establishment to ensure that all levels of trainees are able to get both the appropriate level of training and clinical experience during their rotation through renal services at UHL.
Medical Education in the Leicester Renal Unit
The Leicester Renal Unit has a longstanding commitment to undergraduate and postgraduate medical education. The Unit is actively involved in undergraduate training of students at the University of Leicester Medical School. The nephrology curriculum is mainly delivered through attachments in the General Medicine block in year 3 and the urinary module in year 2.
The department supports a high proportion of trainees to undertake qualifications in medical education. Several registrars have completed a Postgraduate Certificates/Diplomas/Masters in medical Education. A large proportion of the Consultant staff are engaged in senior management roles in Education and Training both locally and nationally.
In addition, a number of innovative training posts have been piloted and developed in the Department: including 2 academic Foundation Year 2 posts, 3 Academic Clinical Lecturer posts. The Department was one of the first to appoint a Consultant/Honorary Senior Lecturer with an interest in Medical Education.
The Unit has strong links with international bodies hosting many overseas Visiting Fellows, International Society of Nephrology Fellows and is an International Sister Centre to a Renal Unit in Nigeria
