Job summary
To be based in the UHL but working within the Primary and Secondary interface to identify and proactively work with the multidisciplinary team. To promote the care provision and the smooth transition of patients in the acute care areas (Emergency Department & Emergency Floor) and ensure the patient is cared for in the appropriate environment.
The practitioner will act as the clinical and professional resource to the multi- professional team, patients and carers within their specialist service.
Main duties of the job
To promote active involvement of patients and their family/carers with discharge planning, keeping them regularly updated throughout the process
To provide discharge support workers with timely updates regarding patients discharge plans, ensuring information is also updated on nerve centre.
To link with the Ward Sisters/Charge Nurses and consultants within each clinical area, to promote discharge planning as high priority - ensuring appropriate and timely discharge of patients at all times.
To link with partner agencies to provide up to date information regarding patients' care needs and escalate concerns for resolution.
To provide education and training for all disciplines involved, with the discharge process.
To monitor hospital discharge delays producing informative performance data to drive the improvement process.
To adapt with ever changing pace of a busy Emergency Department, and adapt to change on a regular basis.
To focus on admission avoidance and place patients on alternative pathways/Signpost to other services to support with high flow demand in the Trust.
