The Children's Medical Emergency Transport service (CoMET), based in the Women's & Children's Clinical Management Group of the University Hospitals of Leicester.
The successful applicant will join a 1:8 rota of middle grade medical staff and advanced practitioners, supporting the paediatric intensive care transport service and cardiac & critical care services at Leicester Royal Infirmary.
CoMET is hosted by UHL and operates from two clinical bases, one in Nottingham and one in Leicester. The successful applicant will be based in the Leicester centre, but will have opportunities to rotate to the Nottingham centre which sites the regional major trauma centre, the paediatric spinal network and regional neurosurgery services.
The post holders will receive training in all aspects of paediatric intensive care and transport medicine including management of cardiac critical care and ECMO patients. The successful candidates may also support mobile ECMO retrievals.
Applicants will therefore need to have completed level 1 training (ST4 and above), and ideally have experience in neonatal or paediatric intensive care at registrar level.
Main duties of the job
- Support the day-to-day running of the clinical activities in the intensive care transport service. You will be part of our established transport team and you will gain experience in transport of critically ill patients by road and by air. You will liaise with referring hospitals regarding retrieval and repatriation of patients. This will include both short trips within the local network and transport of patients to and from hospitals principally within the East Midlands, but our service regularly transports patients to and from anywhere in the United Kingdom and occasionally beyond. The transport of patients for advanced respiratory care (+/- ECMO) will be achieved by conventional means with the possible addition of treatments such as High Frequency Oscillation and inhaled nitric oxide. The most unstable patients and those in whom their current treatment cannot be replicated in the ambulance/aircraft may be transported on mobile ECMO.
- Sustain and develop teaching and research in conjunction with the University of Leicester, and to participate in and fulfil the requirements of the Royal College of Paediatrics and Child health CPD scheme (where applicable) or equivalent. It is expected that trainees will sign up to the RCPCH ePortfolio where they have not aready done so.
The service is consultant led, and often consultant delivered, depending on the clinical acuity of patients and the experience of the middle grade clinical staff.
About us
The regional transport service for the East Midlands has been formed by a collaboration of the existing transport teams based at Nottingham and Leicester.
The service provides:
- An acute transport team operating 24/7/52 with capacity to advise on the management of critically ill children who present to an East Midlands Hospital, stabilise them and transport them to an appropriate PCC facility, in line with NHS England Standards and recognised best practice:
- A repatriation team operating weekdays and during daytime hours only, to safely transport patients to or from PCC facilities, where the level of dependency does not require the full transport service. This will most commonly be 'back transfers' to the referring hospital at the end of an episode of critical illness. The purpose of this service is to enable the most efficient use of expensive and pressurised PCC beds, to deliver care to patients in the most appropriate surroundings, and as close to home as possible:
- Outreach advice and training to East Midland's hospitals in the initial care and stabilisation of acutely ill children, recognising that this improves outcomes and may reduce the need for intensive care admission. This service will be provided in line with NHS England Standards and recognised best practice.
The new service is managerially aligned with the CenTre neonatal transport service. Increasing links with our neonatal colleagues will be formed as the service develops.

- Participate in the weekly pre-operative cardiac surgery forums, monthly clinical practice and clinical risk meetings, morbidity meetings, and other activities relevant to the running of a high quality critical care service.
- Participate in audit activities in the directorate relevant to clinical practice
- Sustain and develop teaching and research in conjunction with the University of Leicester, and to participate in and fulfil the requirements of the Royal College of Paediatrics and Child health CPD scheme (where applicable) or equivalent. It is expected that trainees will sign up to the RCPCH ePortfolio where they have not already done so.
Two consultants are on call at all times for Paediatric Intensive Care, giving the two paediatric Critical Care Units in Leicester separate and independent consultant cover, with a third consultant on call for Critical Care Transport. The service is consultant led, and often consultant delivered, depending on the clinical acuity of patients and the experience of the middle grade clinical staff. Trainee medical staff is therefore well supported in these posts, and there are ample opportunities for teaching and learning.
