Bank Physiotherapist Lancaster Ward

We are looking to employ a Bank Physiotherapist to be involved in an exciting opportunity to offer community rehabilitation and reablement closer to home for our patients of Lincoln.

Lancaster Ward has been identified to build an exemplar model of care, delivering person-centred focused care, demonstrating our strengths of group working across the Lincolnshire health and care system.

The ward will transform to provide step down intermediate, time-limited care for mainly pathway 2 patients; offering short term support to help a person recover until able to live independently or move onto longer term care and support.

Referrals into Lancaster Ward will be through therapy to therapy referral processes, supported by the LCHS Flow Team, who will triage and assess the patient for suitability.

Lancaster Ward will be operating as a community ward in the acute setting.

Main duties of the job

To provide a high standard of Physiotherapy to pathway 2 patients.

To perform more advance & detailed assessments with patients having more diverse & complex conditions, providing a diagnosis and develop and deliver an individualised treatment programme.

To hold own caseload and be responsible for a defined area of the service, working without direct supervision.

Assist in the supervision of Junior staff and physiotherapy students

Undertake evidence based audit and research projects to further own and teams clinical practice within each area and make recommendations to Site/Trust Leads

This will not constitute a formal merger of the two organisations, but will bring the Trusts together under a single Board and Executive Leadership Team, with the goal of improving the care that is provided to patients both in the community and in hospitals across Lincolnshire. Both Trusts will retain their separate statutory names and legal obligations.

The Group will be known as Lincolnshire Community and Hospitals NHS Group (LCHG).
