Job summary

This substantive post provides an exciting opportunity to join our Haematology cancer team and make a real difference to cancer patients and their families.

We are looking for a highly motivated, organised and compassionateperson to join our busy, fast paced Lincoln, Boston and Grantham Haematology cancer team. You will support all patients throughout their pathway following a diagnosis of cancer.

You will have experience of working in a patient or public facing role and an insight into cancer or long term health conditions.

Main duties of the job

The NHS long term plan is for all patients diagnosed with a cancer to have access to personalised care. This includes holistic needs assessment. The role will involve working closely with the Haematology clinical nurse specialists to provide:

  • Administrative and IT support, working competently with a number of hospital IT systems
  • Accurate record keeping and collation of data
  • Frequent dealings with patients and health care professionals, by telephone and face -to -face contacts.
  • Completion of holistic needs assessments and care -plans throughout the cancer pathway
  • Referrals to appropriate sources of support
  • Communicating confidently with all members of the MDT
  • Supporting the the Haematology cancer nurse specialists to deliver individualised care.
