Job summary
The Trust is committed to the provision of a modern high quality, Consultant-led elderly care service, and the successful candidate will be expected to work alongside the 7 Consultants in Elderly Care currently in post.
There are daily consultant ward rounds with the expectation that every patient will have a daily senior medical review. There are efforts to develop enhanced care for frail older people and alternatives to hospital admission, such as virtual Frailty ward.
This post will input into the care of older people's wards, the orthopaedic wards and MEAU, but the service model is in continuous evolution in order to meet the demands of the population. The successful post holder would be expected to participate fully in innovating the service for older people.
In-patient care is undertaken predominantly on the HCOP wards: Scampton (20 beds) and Carlton Coleby (28 beds).
The Frailty team work supporting the emergency department and MEAU, have a dedicated SDEC (Same day Emergency care) unit and a Frailty Assesment Unit (16 beds). They also support Frailty Virtual ward.
The team provide General Geriatric clinics and specialized clinics such as Falls and Movement disorders
Main duties of the job
This is a 10 PA job plan with 1.5 PA's attributed to SPA. It is a provisional job plan and indicative. It may change, in consultation with the Clinical Lead, in the future according to service needs.
There is opportunity to provide educational and clinical supervision for additional PA's. Teaching duties also attract additional PA's. Job plan review should take place after 3 months when the SPA allocation can also be reviewed.

The ward has 28 patients, you usually will be responsible of 14 patients. Sometimes if the other consultant is on leave you will be responsible of the 28 patients (not all of them need your review, mainly new and sick) with a registrar to support.
It is a condition of the appointment that the post holder will be willing to work in any of the Trust's locations or outreach district general hospital clinics.
The Clinical Lead will work closely with the post holder to provide the managerial guidance, initial mentorship and support necessary to deliver the aims and objectives of this appointment.
There will be an annual job plan review by the Clinical Lead and Clinical Business Unit. Annual appraisal will be undertaken in line with trust policy.
