Job summary
Applicants are invited to apply for the post of Living With Cancer (LWC) Acute Project Manager at United Lincolnshire Teaching Hospital. In collaboration with the Living with Cancer Programme, this key role ensures the LWC strategy for Lincolnshire is delivered across the acute trust, whilst also identifying and supporting improvements around the personalisation agenda for cancer patients.
The post holder will play a pivotal role in supporting teams to deliver personalisation across cancer pathways and services by supporting operational and clinical teams with the implementation of the core elements within the Personalised Care agenda as per the NHS Long Term plan, using the comprehensive model of care as a driving force. The post holder will work primarily supporting PSFU (personalised stratified follow up) pathways specifically RMS (Remote monitoring system) to drive positive change.
They will be expected to provide expert leadership, organisational and management skills to ensure the alignment of national programmes alongside regional drivers are met as they work across the system. They will support clinical teams with the development of policies and processes to facilitate the implementation and delivery of improved personalised care and support for cancer patients. Main duties of the job
The post holder will work as part of the Cancer Services team within the acute Trust (ULHT) to support them in delivering the key LWABC priorities, leading on identified secondary care projects specifically PSFU and RMS
To lead on the delivery of the secondary care aspects of the Cancer Recovery Package. These include embedding the Holistic Needs Assessment throughout the cancer pathways, embedding Treatment Summaries, supporting the design of Health and Wellbeing events and personalised stratified follow up (PSFU)
This role will include working closely with the Macmillan Development Manager within LWCCG to ensure a cohesive delivery approach.
This role will include working across the acute Trust with cancer multidisciplinary teams (MDT's), Matrons and Business Units.
This role will require effective communication with wider stakeholders including Clinical Leads and Managers, Macmillan Partnership Quality Lead and patients.
This role will require the ability to meet deadlines and lead on delivery of a defined secondary care projects within the acute Trust.
