Staff Nurse - Lincoln Stroke Unit - Band 5

Job summary

The role will involve being able to care for hyper- acute / acute stroke patients and supporting the Allied Health care professionals in the early rehabilitation stages following a Stroke. There is a program of training to ensure stroke specific competency is achieved and successful candidates will be responsible for maintaining their own competency using the principles of nursing practice to deliver evidenced based care

Main duties of the job

We are looking for someone to join our enthusiastic team in this exciting new challenge to take the stroke service forward and ensure that we all have the knowledge and skills to give a high standard of care to Stroke patients and support to family and those caring for stroke patients on discharge.

We are a twenty eight bedded Stroke unit providing hyper-acute, acute and rehabilitation as part of an interdisciplinary team to patients who have suffered a stroke. This is an exciting time in the development of the stroke services for patients in Lincolnshire and the successful candidate will be an integral part of taking the stroke service forward.

Job responsibilities

Consistent delivery of a high standard of nursing care to Stroke patients maintaining privacy and dignity at all times. Through assessed, planned, implemented and evaluated care meeting identified care needs through goal setting and MDT meetings.

Act as mentor or an associate mentor to pre-registration students, newly appointed staff, and HCSWs working towards NVQ, or on induction for hospital bank staff.

Be conversant and comply appropriately with all trust policies and protocols. Attend mandatory study days.

Ensure the promotion of health and safety of patients, visitors and staff using correct aids for manual handling.

Assist to co-ordinate and monitor the progress of patients along the integrated care pathway for Stroke patients, analysing and reporting variances from pathway to relevant MDT members.

Assist to maintain nursing practice of the unit of both self and others that complies with trust policies and protocols, national acts, standards, frameworks and guidelines and professional code of conduct. Ensure that all mandatory and stroke specific training is completed annually or as required and that your stroke unit based Personal development folder is kept up to date

Assist to develop, introduce and apply research based findings to nursing practice relevant to stroke care. Assist in research based studies being carried out on the unit.

Communicate effectively to all members of MDT the goals and identified care needs of a group of patients within the Stroke unit supporting the implementation and maintenance of MDT working within the unit.

Maintain and demonstrate clinical skills and knowledge taking responsibility for personal professional development specific to Stroke. Develop skills and knowledge to ensure able to care for Stroke patients in hyper acute, acute and rehabilitation stages of their inpatient care. This includes developing skills to assessing caring and management of patients who have been thrombolysed.

Take responsibility as Stroke unit co-ordinator for the running of a shift.
