Job summary

This is an exciting opportunity for an experienced Occupational Therapist to join our Acute Rehab Coordinator (RC) team working alongside our existing Physiotherapist RC and Consultant.

You will work as part of a defined team coordinating the Multidisciplinary team (MDT) involved in the care of patients requiring acute rehabilitation, in particular Major Trauma patients working closely with the Acute Team Rehabilitation Consultant to coordinate clinical and discharge planning for patients with in Hull University Teaching Hospitals NHS Trust.

The maintenance and development of clinical expertise and continuing professional development is essential and will be demonstrated by holding a clinical caseload within the specialty of acute trauma and / or rehabilitation. There will be a requirement to work 1 in 5 weekends in conjunction with Therapy Services.

Main duties of the job

Responsibilities include ensuring the completion and submission of Rehabilitation Prescriptions (RP) for complex patients and implementation of the rehabilitation pathway, you will provide expert clinical opinion to patients with complex need in any specialty within the organisation.

To act as the nominated deputy to the Rehabilitation medicine Consultant, undertaking initial assessments and coordinating transfer of patient on to the rehabilitation unit where appropriate. Coordinate the transfer of care for those patients who are repatriated outside of the Trust. You will also work closely with therapists in the organisation and both attend / host MDT meetings. Required to work closely with the TARN team ensuring accurate information is recorded and Best Practice Tariff is achieved

The post holder is required to act as key worker for patients with specialist rehabilitation needs whilst in the acute phase of care until they transfer to a rehabilitation facility or alternative provider either internal or external to HUTH. They will also provide clinical leadership through the development if therapy staff involved in acute trauma rehabilitation and will also be required to lead evaluation and development of clinical practice in order to demonstrate compliance with national and local guidelines.

The post holder will undertake a clinical interventions providing support to Therapy Teams across the organisation

Experience
