Job summary
Hull University Teaching Hospitals are seeking to recruit a small team of Band 3 Therapy & Nursing Associate to join a small team who work in a innovative role spanning both the therapy and nursing professions.
This role aims to ensure that the ongoing health and social care needs of patients admitted to acute are are fully understood and addressed from the point of admission to the point of discharge.
The successful applicant will be supporting existing staff with the assessment of need across a range of clinical conditions (many of which present with varying degrees of complexity), the management and maintenance of basic care routines, progression of mobility and transfers, and delivery of evidence based interventions to facilitate recovery, promote well-being and optimise independent functioning as far as reasonably possible in all areas of life.
There will be a particular focus on supporting patients, carers and families in relation to transfer of care, supporting with the identification and adoption of appropriate pathways to facilitate timely and effective discharge from the acute care setting.

Main duties of the job
We are looking for an enthusiastic team member to complement our existing nursing and therapy workforce by providing a hybrid of support to complement the services currently provided by both these aspect of health care.
This is an excellent opportunity for an enthusiastic person to gain experience working as a valued member of the multidisciplinary team, providing support to Physiotherapists and Occupational Therapists, Dietitians & Speech & Language Therapists, Nursing Staff and Discharge Assistants, in carrying out delegated duties. In addition, the post holder will assist in undertaking clerical and administrative duties directly related to patient care including making of onward referrals, completion of patient documentation, ordering of mobility aids and other clinical supplies and any required data collection
We offer excellent in-service training and close supervision will be offered in all clinical areas. Personal and professional development is a high priority and the successful candidate will be expected to demonstrate a commitment to the development of new skills associated with this innovative role.
