Job summary

We have a great opportunity for a registered nurse to join ourIntensiveSupport Team (IST), withinWealden Neighbourhood Mental Health Team,Specialist Older Adults Mental Healthservicebased at MillwoodinUckfield, East Sussex.

You will be a key part of our multi-disciplinary service for older adults with mental health problems, including Dementia.

This post will involve working with olderpeople with complex mental health needs as well as physical health care needs. You will be expected to support both the client and theircarers and be able to work cohesively with other services both within our Trust and partner agencies.

You will need to be caring, compassionate and a good communicator, with a commitment to improving the lives of our service users through creative,evidence based practice. As a Band 6 Nurseyou willbe responsible for working collaboratively with service users and their carersto create care plans that meet their needs and maximise their independence.

Main duties of the job

The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:

To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk.

To develop alongside the service user their care plan.

To offer specialist nursing assessment and advice whererequired.

To assess and manage on going risks asidentifiedduring the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management ofthemas necessary.

To provide supervision of junior staff and trainees whereappropriate.
