Job summary
We are pleased to be able to offer an exciting opportunity to join the team at Health in Mind as our Primary Care Mental Health Senior Practitioner - Referral Coordinator. This is a 1 year Fixed Term (37.5 hr) Maternity cover post with an expectation of some out of office hours working (see below).
Health in Mind is an NHS Talking Therapies service that offers a primary care mental health service that supports people in East Sussex, excluding Brighton and Hove. The PCMHP's within Health in Mind provide a critical function in triaging referrals into the service, providing timely and consistent assessments, brief non-NHS TT interventions and onward referral as required. Liaison with key stakeholders and third sector organisations also forms part of this role. Although you will be working primarily in the Eastbourne, Hailsham and Seaford area the ability to travel across sites in the East Sussex area (excluding Brighton and Hove) to meet the overall demands of the service is required.
Main duties of the job
It is essential that you are a registered mental health professional or equivalent (see job description). Main duties include: ensuring that all referrals into Health in Mind are triaged and managed appropriately in accordance with agreed time frames and data available; assuring high quality screening of referrals and ensuring an appropriate and clinically sound response; overseeing the work of other team members working as part of the Triage rota to ensure high quality practice and adherence to agreed standards and KPI's.
The successful applicant should have excellent clinical skills in assessment, risk assessment and as well as strong communication skills. You will be working closely with other professional groups within the team and also externally, in particular the Triage team, Triage Administrator and Triage Hubs, PCMHP Team Leads and IAPT staff team. Providing advice and support to your colleagues is an integral part of this role. The ability to develop and maintain strong relationships with stakeholders, such as GPs, secondary mental health services and non-statutory organisations is vital. The post holder will also contribute to broader Service and Trust developments in relation to our Triage team, local Triage hubs and PCMHP role developments in Sussex.
