We have a great opportunity for a full-time band 3 community support worker to work in Intensive Support Element of the Dementia & Older People's Mental Health Team based in the Crawley and Horsham area and will be covering Crawley and Horsham areas in West Sussex. Intensive Support operates an 9am - 5 pm shift pattern, with a 30 minutes break, Monday to Fridays, 9am - 5pm at weekends and bank holidays.
The ability to travel is essential in this role, driving is also essential.
Main duties of the job
The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work.
You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults.
