Job summary

Do you want a new experience and open to learning then apply for our band 6 post. We are seeking someone who is passionate about working with complex duel diagnosis within SWIFT mental health and Drug and Alcohol theme. You will need to hold RMN/ RN/ Social Worker or OT professional qualification and have a minimum of 3 years post qualifying experience. You will be joining a long established team of Nurses and Social workers who work in SWIFT Mental health and Drug and Alcohol team. This is an exciting opportunity for anyone wishing to further develop their existing skills and knowledge and be part of parental mental health and substance misuse teams. You will receive additional training to equip you to complete the complex assessments we provide to Children's Social Care to enable best practice for this post.

This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).

Main duties of the job

The main duties of the Job are to assess parental mental health and substance misuse the risk and impacts of such to children's well being and development, informed by the safeguarding threshold of children open to ESCC. You will formulate recommendations on your assessment and deliver interventions that are bespoke to the Parent/ Carer/ Guardian.. You will work along side East Sussex Children's Social care to assist with the formulation of risk and safe management of families. We take a whole family approach to our work and often joint work with other Themes within Swift such as Domestic abuse/ Sexual risk/ Psychology. Adering to the integrated joint management protocol for SPFT and ESCC.

About us

SWIFT is a jointly commissioned service offer between ESCC and SPFT NHS. The service specification governing assessment delivery is informed by the safeguarding threshold of the case and the referral presentation of the parent/carer or child. Cases will be prioritised based on clinical need and safeguarding threshold. SWIFT takes a whole family approach to risk assessment of Mental Health /Substance misuse,/Domestic Violence and Sexual risk to children and young people focusing on assessing the risks of the carers to the child/children in their care. These assessment of the adults provide East Sussex Children Services locality teams robust information that assist the family assessment/ safeguarding process under children's care act.

Job responsibilities

  • To assess and respond to identified risk. To provide a response to priority referrals. To reduce and manage risk in consultation with the SWIFT management team. To communicate information regarding identified risks to colleagues in other services.
  • To participate in the safeguarding process through attendance / contribution to core groups, case conferences and MDT meetings and care proceedings including attendance at Court when required.
  • To Manage a case load of complex adults with multi faceted concerns related to poor Mental health and Substance misuse.
  • To apply professional skills across the speciality related to your qualification.
  • To provide consultation and training to practitioners within Childrens Services and Adult Services on the impact of mental health / substance misuse / on parenting.
  • Duties will involve the delivery of interventions in accordance with the agreed SWIFT Service Specification, Service Standards and Clinical Policies and Practice Standards Documents.
  • To adhere to Service Specification, Clinical Policies, procedures and Service Standards for ESCC and SPFT and the agreed joint services working protocol for SWIFT.
  • To provide supervision to junior members of staff as required.
  • To facilitate appropriate support to family members/carers.
  • To adopt effective health promotion, harm reduction methods of intervention for families accessing care.
