Job summary

Would you like to come and work with the Assertive Outreach Team providing high quality clinical care for patients across the Horsham, Crawley and Mid Sussex areawho experience psychosis?

We are based at New Park House in the beautiful town of Horsham, just a stone's throw from Horsham Park and the mainline train station.

We are looking for a Support Worker to take on this role with the opportunity to be supported to progress to the associate nurse training role through further training and development.

Main duties of the job

You would be a key member of the specialist multi- disciplinary team providing support in delivering individual care through case management of caseloads. Also, you will be working with individuals on specific aspects of their support plans.

We are looking for an enthusiastic creative person who has an interest in working with this patient group, who can demonstrate a clear understanding of team work.
