We have a brand-new opportunity for a Band 5 Mental Health Nurse, Social Worker or Occupational Therapist with mental health experience to join the Acute Day Service, here at Langley Green Hospital.
The Acute Day Service offer service users in a mental health crisis, four weeks of psycho-educational groups to help learn coping strategies and self-manage their mental health.
Main duties of the job
As a Community Mental Health Practitioner in our team you will meet with service users; provide a care plan, review their mental state, run the therapeutic groups. You will be responsible for monitoring and developing treatment plans in order to make progress with service users and ensure effectiveness of interventions.
You will be directly involved in developing discharge plans in collaboration with service users and their families, as well as occasionally representing the clinical team at multi-disciplinary meeting and clinical reviews of patient care.
You will work very closely with the CRHTT (Crisis Resolution & Home Treatment Team) and our inpatient wards.
