You will be working alongside a group of experienced and highly motivated clinicians who will support you wherever you are in your career journey while we focus on the rehabilitation pathway for our patients.
The post holder will participate in the Leadership Team and will be working collaboratively with the Consultant, and managers to achieve the most efficient and effective use of resources.
This post will help to develop better provision and capacity for Rehabilitation Services within SPFT while allowing access to the Forensic Healthcare Service (FHS). There is a strong focus on working in partnership with families at this service. We include patients in planning their care, and we value and embrace their unique qualities.
Main duties of the job
The unit comprises 8 inpatient beds in a purpose-built area, along with 16 individual flats.
This allows patients to move along the rehabilitation pathway.
The post holder will:
- Have autonomy to make decisions to improve patient outcomes. Partnership working withrehabilitation services, the Rehab hub and SAM Team.
- There may also be necessity to work with colleagues in Probation, Police, Court, Social Services, voluntary organisations/agencies to assist in the management of mentally disordered offenders who have rehabilitation needs.
- Work with colleagues in adult mental health services.
- Assess patients who are referred to 78.
- Participate in weekly referrals meeting/rehab hub calls
- Clinical leadership role within the multidisciplinary team and across the service as a whole and work with colleagues and management to ensure optimal service delivery.
- Participate in business planning for the locality and, as appropriate, contribution to the broader strategic and planning work of the Trust.
- Work alongside the team helping to lead the improvement of the quality of care within the team and contribute to improving quality across the system.
- The post holder will be encouraged to contribute to other relevant management activities within the Directorate and Trust. This might include participation in clinical governance activities, relevant working groups
