Community Mental Health Practitioner - East Sussex - Band 6 - Full-Time permanent post

The post is based with the Assessment and Treatment Service at Hill Rise in Newhaven, which is a fishing town on the Sussex coast that sits in the beautiful South Downs National Park. The vibrant city of Brighton is just a short drive away and there are regular train links to London. You can even get the ferry to France if you fancy a trip.

Main duties of the job
The Hill Rise Assessment and Treatment Service is part of a lively multi-disciplinary service for adults with severe mental illness. The role is all about improving outcomes for this patient group, working creatively with patients and their Carers to co-create care and risk plans that will help them achieve their goals and to live as independently as possible. This key to success is often about building effective partnerships with GPs, Crisis services, housing and other third sector providers or we might decide to refer patients onto one of our more specialist services, such as our Early intervention in Psychosis or Eating disorders.
You must be enthusiastic, self-motivated, flexible and able to work as part of multidisciplinary team and demonstrate a commitment to the recovery model.

To offer advice on management of patients with mental health issues.4. To ensure mental health and psychosocial assessment, care and support of each client is appropriate to individual needs.5. To map community resources for people with mental health problems8. To work with third sector partners to promote integrated care pathways9. To keep patient records up to date with timely, accurate and comprehensive information10. To produce and keep up to date risk assessments11. To provide clinical advice / consultation and staff support to other health professionals as appropriate within the community.12. To know how to access appropriate services and develop effective networks and channels of communications.13. To adhere to a code of Professional Conduct.14. To attend case conferences & meetings as relevant to the job description.15. Work in accordance with Sussex Partnership Foundation NHS and East Sussex Hospital trusts policies, guidelines and protocols.
Managerial1. To monitor and evaluate functioning of the service by maintaining meaningful statistics that reflect work undertaken.2. To assist in the selection of staff and to participate in their induction as required.3. To supervise students and band 5 staff if required.
Educational1. To identify ones own training needs through a Personal Development Plan (PDP). Access relevant courses/conferences nationally or locally2. To demonstrate awareness of research and its applications to clinical practice.3. To participate in monthly clinical supervision and weekly team supervision.
