Job summary
We are seeking to recruit a passionate and knowledgeable Community Mental Health Nurse to join our Dementia and Older Persons Mental Health Team based in Horsham. We would also welcome applicants with Lead Professional backgrounds in Social work or Occupational Therapy. Alongside non-NHS professional backgrounds, who possess an undergraduate degree with relevant clinical skills, mental health training and community experience which meet the criteria for a Lead Professional role within DOPMHS.
The team is multi disciplinary and very supportive with daily zoning meetings and weekly MDT meetings. We aim to keep people at home maximising their independence and well being. You will be joining a friendly, supportive and creative multi-disciplinary team, dedicated to providing a high-quality service.
This post offers an incentive or relocation package up to the maximum of £4000 (pro rata applies and subject to terms and conditions).
Main duties of the job
You will undertake assessments with older people with mental health problems and people of any age with a dementia , develop a are plan with them in order to provide support and treatment, supporting and guiding their carers and families alongside. You will work alongside colleagues in social care and Primary care as well as voluntary sector organisations to ensure that patients and their families get the help they need and are able to recover and be as independent as possible for as long as possible.
You will assess and visit people in their own and homes and care homes. You need to be able to travel between the office and such visits . The ability to travel is essential in this role.
