This is an exciting opportunity to join our hospital discharge team based at Mill View Hospital. The team is formed of mental health nurses, social workers, bed manager, discharge co-ordinator, and support workers. The discharge team supports patient flow through acute in-patient care using the Lets Get You Home Framework and the SAFER discharge model.
Main duties of the job
The role of the Pathway worker is to strengthen links between In-patient services (NHS and private providers)and the Assessment and Treatment Service, (Community mental health team). The postholder will work collaboratively with both the acute and community clinical teams to facilitate effective discharge planning, supporting communication and planning about a patients discharge pathway. This post will be supporting the West assessment and Treatment service (ATS) also based at Mill view Hospital. There is another pathway worker supporting the East ATS.
We are looking for an experienced, highly motivated, enthusiastic and resourceful community mental health nurse, Occupational Therapist or Social Worker. You will have excellent communication skills and be skilled in liaising with and working across a number of services.

- Pathway Workers will be incorporated into the East and West ATS hubs to provide an interface between the acute wards, CRHT and the ATS to determine pathways for patients on discharge to the community.
- The Pathway worker will advise clinical teams on the appropriate pathway for all patients based upon clinical cluster and assessed needs to be determined through collaborative clinical reviews with acute and CRHT MDT.
- To support acute services to ensure that referrals are made to ATS for all patients who are previously not known to services and will require a 7 day follow up and management by the ATS.
- The Pathway worker will identify patients with complex needs who will require allocation of a Lead Practitioner and specific discharge planning, Housing Options referral, Safeguarding concerns and ensure allocation of appropriate members of the wider ATS.
- The Pathway worker will attend MDT reviews for all acute wards and CRHT on a weekly basis to ensure that updates and pathway management are supported and discharges are not impeded and obstacles are identified and resolution is achieved.
- The Pathway worker will ensure that patients are informed of the pathway to the community and that they are supported to develop appropriate crisis plans on discharge
- The Pathway worker will respond to acute wards and CRHT where issues related to capacity are effectively managed and that the CRHT caseload is appropriate and patients are not delayed in their pathway to the ATS.
